Form Category: Withdrawal Form

Occasionally, a parent and student may determine that SCVCS is not the better alternative for the education of the student. This decision should be discussed with the student’s counselor/teacher. If the decision is made that the student should withdraw, please download and complete the withdrawal form and return to the student’s counselor/teacher or the SCVCS Registrar (registrar@scvcs.org). The name of the new school must be on the withdrawal form.
